STEPS IN GETTING LOG FROM ETIQA

If you are a member of Etiqa, contact PCC of Etiqa to request for teleconsultation.

PCC will inform the member on the requirements needed for Teleconsultation, evaluate the eligibility and initial complaint of member. Here are the following requirements:

• eTiQa membership card
• One (1) government ID with Picture
• Internet connection
• Zoom app or Viber

Note: If the reason for Teleconsultation is deemed to be an emergency case or medically critical case, patient will be advised to go to the nearest emergency facility for a face-to-face consultation.

If member is eligible for Teleconsultation, PCC will issue Letter of Guarantee (LOG).

Once you have your Letter of Guarantee, you can now schedule an appointment with any of our RACE doctors. Please don't forget to sign your LOG before registration.
